Richmond Community Healthcare in Partnership: New outcome based commissioning contract

Local people in the borough of Richmond will benefit from more joined up, out-of-hospital care services for adults with the launch of a ground-breaking new partnership between Richmond GP Alliance (RGPA) and Hounslow and Richmond Community Healthcare NHS Trust (HRCH). 

As part of the new initiative – known as Richmond Community Healthcare in Partnership – Richmond GPs and community NHS clinicians will be working together more closely than ever before to deliver better care for patients.  

The collaboration brings GPs from Richmond GP Alliance - a partnership of all GP practices in the borough - and HRCH’s adult community health services together as part of a new outcomes based commissioning contract which will re-design adult health services and deliver improved outcomes for patients in Richmond.    Find out more here.
