Paediatric Community Nursing Service (Hounslow)

The Hounslow Paediatric Community Nursing (PCN) team are specially trained nurses, healthcare support workers and administration staff who support children and young people in their own home or community settings such as clinics and schools.

The service comprises:

We aim to reduce the impact of childhood illness and reduce the need for hospital admissions. When hospital admission is needed, we work to reduce the length of stay whenever possible.

The nurses are skilled in managing a wide range of complex conditions, which allows the child to spend as much time as possible at home with their family or at school.

They will support children with long term conditions such as childhood cancer. They will also see children who require short term care for example following surgery, help to manage their asthma or achieve continence.

Who we see

Children and young people, who are registered with a Hounslow GP and who have:

  • acute and short-term conditions

  • long-term conditions

  • disabilities and complex conditions, including those requiring continuing care and neonate

  • life-limiting and life-threatening illness, including those requiring palliative and end-of-life care

What we offer

  • Skilled care - including use of asthma inhalers, procedures such as injections, and wound care 

  • Symptom Control Management – if the child or young person requires a period of regular assessment and monitoring of symptoms, for example pain control in children requiring palliative care, exacerbation of chronic conditions such as eczema and constipation. The team also support oxygen dependant babies and children with complex healthcare needs

  • Education – if the family or carer requires training to enable them to continue providing support and on-going care, for example enteral (through the nose or tummy) feeding, suctioning, oxygen therapy, tracheostomy care and administration of medication

  • Nursing procedures – that can be safely undertaken at home

  • Specialist resource – for other health care professionals

Monday – Friday 8.45am – 3.15pm (term time only)

Children’s Community Nursing and Continuing Care
7 days a week 8am – 6pm

Anyone can make a referral. We provide services for children and young people who are registered with a Hounslow GP and are aged:

  • - 0-18 year old - Asthma and Wheeze, Children’s Community Nursing and Continuing Care services teams

  • - 2 – 18 years - Paediatric Bladder and Bowel team

  • - 3 – 19 years attending a special school in Hounslow - Special Schools Nursing team

  • Once a child has been referred, a nurse will undertake a full nursing assessment and agree a plan of care with you and your child.

  • Some children may have needs that our team cannot meet, in which case we will refer them to the appropriate service.

  • We will make every effort possible to book visits in advance at a time and in a setting convenient for you. This could be at home, in a clinic or at school. Occasionally our nurses might be delayed or our clinics may overrun. If we expect to be more than 30 minutes late, we will let you know.
