The Primary Care Patient Coordinator (PCPC) service provides proactive, coordinated support to general practices across the London Borough of Hounslow.
Our practice-based coordinators work with practices to ensure they have effective processes in place to support their patients with long term conditions, such as diabetes, asthma and mental health conditions.
The coordinators also help practices to make sure children are up to date with their immunisations, women have had their cervical screening, and that eligible patients have their flu jab each year.
PCPC also supports multiple teams and services to work together to enhance the care provided to patients with complex needs.
To do this, the coordinators organise monthly multi-disciplinary team meetings in each practice, which involve community nursing, social services, GP practice staff and other specialist teams as required. The coordinators minute the meetings and make sure that actions have been completed.
This service is not patient facing and does not take referrals. However we are commissioned to work with all practices so please speak to your practice if you would like to find out more about how PCPC supports them.
