From 1 October, HRCH and Your Healthcare will be bidding a fond farewell to the Adult Continuing Health Care service and Children’s Continuing Care service, which is transferring to the NHS South West London Integrated Care Board Kingston and Richmond Place Team (SWL ICB) and known as the All Age Continuing Care (AACC) service.
Following the transfer, the AACC team will lose access to the SystmOne electronic patient record and the London Shared Healthcare Record and so will need to contact teams directly for supporting information as and when needed. Your support with this is greatly appreciated.
