Falls and bone health (Hounslow)

The Hounslow Falls Prevention and Bone Health Service aims to reduce the rate and risk of falls and fragility fractures amongst Hounslow adults, and to improve the health and mobility of older people and sustain their independence.

The service is designed for people aged 65+ who are felt to be at risk or afraid of falling or have had occasional or recurrent falls.

The service aims to work preventatively, helping people to understand what makes them more likely to fall and how to reduce their risk of falling.

The service will also see those who are aged 50+ for bone health management where there is an increased risk of fragility fracture.

The service works in partnership with the Frailty Team at West Middlesex Hospital and the Home Safety Team within the London Borough of Hounslow in order address all needs related to falls and bone health whilst supporting longer term maintenance and self management.

The overall aim is to help stop people from falling in the future and reduce hip fractures and other injuries.

If you are worried about falling, please visit your GP. If your GP is concerned that you may be at risk of a fall you will be referred to the Falls Prevention Clinic for a more detailed assessment.

Referrals should be made using the HRCH referral form via the Single Point of Access service.

Patients must have a Hounslow GP to be referred to this service. We will also accept people who live in the borough but whose GPs are based elewhere
