The Community Recovery Service is an integrated health and social care service for adults identified with uni- and multi-disciplinary recovery needs.

The service supports independence by providing rehabilitation and reablement after an acute illness, injury or change in life circumstances.

It aims to restore, maximise or prevent deterioration in physical and social functioning through episodes of rehabilitation and Reablement

The team comprises:

  • Assessors

  • Neuropsychologist

  • Occupational Therapists

  • Physiotherapists

  • Rehabilitation Assistants

  • Social Workers

  • Specialist Nurses

Referrals to the service can be made by GPs or other healthcare professionals, hospital teams, social services, or the voluntary sector. People can also self-refer to the service.

All referrals to the service from GPs or other health and social care professionals should be made using the HRCH single referral form (Hounslow) via the trust's Single Point of Access (Hounslow) service

The service appreciates the inclusion of discharge summaries, clinic letters and reports as appropriate.

Any health and social care professional or voluntary agency can refer people to this service.

To access the service, people must:

  • Be aged 18 or over

  • Be registered with a Hounslow GP or resident in London Borough of Hounslow

  • Require rehabilitation and reablement after an acute illness, injury or change in life circumstances with a primary physical or social need. 

The service functions from 8am to 6pm every day, including weekends and bank holidays.
