This is the Single Point of Access (SPA) for healthcare professionals to use to refer adults with Richmond GPs to the following community-based services:
- Intermediate Care Team
- Community Physiotherapy
- District Nurses including specialist services
- Community matrons
- Community Neuro-rehabilitation Team
- Falls Service
- Cardiac Rehab Service
- Respiratory Care Team
- Richmond Response and Rehabilitation Team
- Desmond Diabetes
- Speech and Language Therapy
The SPA team triage all referrals and ensure that they are directed to the appropriate service.
This service is for GPs and other referrers only.
